I hereby authorize the staff of the Jersey Shore Elite Basketball Clinic to act according to their best judgement in an emergency requiring medical attention. I hereby waive, and release Markham Place School Little Silver, NJ 07739, Jersey Shore Elite Basketball Clinic and staff from any liability for any injury or illness suffered prior to or while at camp. I have no knowledge of any physical impairment that would effect my child’s participation in the camp program.I hereby agree to assume complete financial responsibility for any personal injury or property damage incurred as result of a willful or negligent act of my child while he/she is a camper at the Jersey Shore Elite Basketball Clinic or at Markham Place School Little Silver, NJ 07739. I hereby agree that in the event my child is unruly and the appropriate officials of the Jersey Shore Elite Basketball Clinic are of the opinion that such child may harm himself or others, or damage property, they can be terminate such child as a camper and I agree to arrange for such child’s immediate transportation home, without any refund.
